We recognize and participate in a number of insurance plans. Not all requirements and guidelines are the same. It is the patient's responsibility to understand their individual plan. Some plans require the following:

• Referral authorization from your primary care physician.
• Using designated hospitals, laboratories or X-Ray facilities.
• Obtaining pre-certification approval for hospitalization or surgery.
• Waiting period for pre-existing conditions.

Not following the above guidelines could result in your insurance company denying or delaying payment, which leaves the patient responsible for the services. If you do not understand the requirements of your plan or have any questions, please contact our business office, and someone will help you.

Co-payments, deductibles and any service not covered by your insurance plan will be the responsibility of the patient.
